Case Manager - Youth Advocate Shelter

Youth Advocate Shelter Case Manager

Location: Lowell

We are seeking a Youth Advocate Shelter Case Manager to provide services to youth placed within the 24/7 Crisis Transitional Youth Shelter. The Youth Advocate Shelter Case Manager is responsible for providing case management services to Youth and Young Adults (YYA) ages 18-24, using a trauma informed approach that includes: developing monthly Rehousing Plans, referrals to community resources, advocacy, assessments, and crisis intervention, with the goal of placing YYA into permeant housing.

AVAILABLE SHIFTS INCLUDE:

  • Monday-Friday 8:00 am to 4:00 pm
  • Monday-Friday 12:00 pm to 8:00 pm

Responsibilities include:

  • Complete intake and needs assessments with each YYA, which may involve a broad range of areas including housing, finance & budgeting, employment, education, food & nutrition, legal issues, health, and daily living skills.
  • Interview and evaluate clients to formulate Individualized Service Plans based on Program requirements.
  • Assist YYA’s in securing permanent housing by making full use of all available housing opportunities including the private rental market and reunification where applicable
  • Work closely with YYA to identify and build upon strengths and develop strategies to address barriers and concerns identified through the assessment process.
  • Maintain compliance standards with Youth Services programming.
  • Help YYA build self-sufficiency plans.
  • Assist YYA to transition to self-advocacy roles.
  • Provide emergency response management.
  • Establish and monitor goals.
  • Work collaboratively with interdisciplinary team of collateral providers.
  • Maintain accurate case records.

Qualifications:

  • Prior related experience required
  • High School Diploma or Equivalent required
  • Prior assessment, advocacy and case management work in a Human Service field.
  • Knowledge and experience working with low income, culturally diverse, and at-risk individuals.
  • Demonstrated knowledge of community resources available to low-income individuals.
  • Excellent customer service, organizational, oral and written communication skills.
  • Ability to manage multiple priorities and work in a fast-paced environment.
  • Proficient computer skills required in basic computer software and data collection systems.
  • Excellent crisis management skills.
  • Ability to maintain accurate records and confidentiality of information.
  • Excellent attention to detail.
  • Valid Driver’s License
  • Bilingual (English/Spanish, English/ Haitian-Creole, English/Khmer, English/Portuguese) preferred.
